Consent
By submitting this form I consent to Bloom From Within collecting, using, and disclosing my child's personal, educational, and health-related information to schools, government agencies, and health professionals for educational support, welfare, and legal compliance purposes, in accordance with the Education Act 1990 (NSW), Privacy and Personal Information Protection Act 1998 (NSW), and Health Records and Information Privacy Act 2002 (NSW). For students aged 14+, I acknowledge that NSW law recognises their developing decision-making capacity, and Bloom From Within may maintain confidentiality at their request unless there is a serious risk to health/safety or a legal obligation to disclose (Children and Young Persons (Care and Protection) Act 1998). This consent remains valid until withdrawn in writing, except where disclosure is legally required.
